Delirium is a very interesting Runeword, due to its unique power: a 1% chance to change the wearer into  Level 50 Bone Fetish when struck. And what could be more interesting than that?

The Delirium Runeword is the only Runeword that grant this mod.

Delirium is a spell, and when it triggers, will transform the wearer of the helm into a level 50 Bone Fetish for about 1 minute.

Bone Fetishes are the small, quick little undead found running underground in Act 3, who are well-feared for the damage they can cause when they explode.

Fortunately, your character, as a Bone Fetish, won’t be exploding.

The drawback to being a level 50 Bone Fetish is that you cannot cast spells, therefore this is a huge penalty for casters (except the Druid, who is still able to cast his Armageddon spell while in Bone Fetish form). The only attack you can do as a Bone Fetish is a melee attack: a head butt. Therefore, this isn’t that big of a drawback for melee characters.

Note: A Druid can transform back from the Bone Fetish form by using the Werewolf or Werebear Skills.

11% Chance To Cast Level 18 Confuse On Striking

These mods can trigger quite often, and when you foes are incapacitated with Mind Blast, Terror or Confuse, it helps because you character is then: not getting hit and your foes are not as effective in killing you.

+2 To All Skills

Only 3 Unique helms have this mod: Harlequin Crest, Nightmare’s Veil and Andariel’s Visage. Those helms, especially Harlequin Crest, are more attractive to players due to the greater usefulness of the mods on them.

However, if you don’t have any of them and still yearn after the +2 To All Skills, just don a helm with the Runeword Delirium in it. 

25% Better Chance of Getting Magic Items

A  nice bonus for your MF runs.

Summary:

The main reason for players creating this Runeword in a helm is because they don’t have Harlequin Crest, Nightmare’s Veil or Andariel’s Visage yet, but still want the +2 To All Skills that Delirium grants.

There are some players who find this a useful Runeword for the helm being used by their Mercenary, especially on an Act 2 Mercenary with the Jab Skill. This is because of the many potential Confuse, Mindblast and Terror triggers, which will definitely help the Mercenary remain alive for a longer period of time.
